Chelan County PUD
Third Party Consent Form - Click Here

From PUD Form:

The Third Party notification is intended to aid in preventing unnecessary disconnection of service. If you care for a relative, friend or client who could overlook paying their bills due to illness, financial hardship, language difficulties, or other issues, this program can help. It allows Chelan County P.U.D. to send duplicate bills and/or notices to a designated person. The role of the third party is to assist the District’s customer in understanding what must be done to resolve a problem. This service is available to all customers with the written consent of the customer. Responsibility for payment of the bill remains with the customer of record.

Third party notification can also be a tool to provide usage information to a designated person monitoring energy use for a commercial customer or for assurance that a bill paying service has paid the bill.

Recently one of our RAWV members brought to our attendtion that the Chelan County PUD has changed their criteria for new applications for power service.

In the past, less information was necessary for new applicants applying for service. Now, if a person signs up over the phone (even if it’s a long time customer and long term tenant), the PUD requires a verification of SSN. If a prospective customer or existing client walks into the PUD office, the PUD will check an acceptible ID (from a list of acceptable documents that will be available soon on the website) and they will ask for the SSN, BUT THEY WILL NOT REQUIRE IT for walk in requests.

From the internet:
The Fair and Accurate Credit Transactions Act of 2003 (FACT Act or FACTA, Pub.L. 108-159) is a United States federal law, passed by the United States Congress on November 22, 2003,[1] and signed by President George W.
Bush on December 4, 2003,[2] as an amendment to the Fair Credit Reporting Act. The act allows consumers to request and obtain a free credit report once every twelve months from each of the three nationwide consumer credit reporting
companies (Equifax, Experian and TransUnion). In cooperation with the Federal Trade Commission, the three major credit reporting agencies set up the website, annualcreditreport.com, to provide free access to annual credit reports.[3]

The act also contains provisions to help reduce identity theft, such as the ability for individuals to place alerts on their credit histories if identity theft is suspected, or if deploying overseas in the military, thereby making fraudulent applications for credit more difficult. Further, it requires secure disposal of consumer information.
