There are also many other forms that you may choose to add to this, depending on your unit or the situation.  For example there are forms for septic, air conditioning units, lawn care, pets, etc. It is always a good idea to do more if they apply to your situation. Remember that the  tenant should receive copies of anything they sign at the time they sign them.
